The New Vision Health & Wellness Philosophy
The modern American Health Care system is broken. Medical Care is episodic and fragmented. Emphasis is placed on “sick care” and as a society we focus very little on the prevention of disease and the maintenance of wellness. The system is complex and has many potential pitfalls for the people who depend upon it. There does not seem to be a comprehensive approach that binds it all together, until now.
New Vision Health & Wellness is based on the platform of a Patient-Centered Medical Home. Our doctors are committed to providing you the best possible service following the mandates of this nationally recognized and growing movement. Our commitment is to help you navigate this system by providing you the very best in Foundational Care.
We believe the distinction between Foundational Care and the current Primary Care & Referral approach is traditional primary care's lack of an in-depth single provider approach to Chronic Disease State Management. Given the constraints of time and the current system, patients often have multiple doctors involved in their care. This has the potential to create situations which can lead to negative outcomes for patients.
New Vision doctors strive to be your “medical quarterbacks,” guiding all of your care, comprehensively, safely, with the goal of achieving positive health outcomes for you. Our professionals will serve you in the outpatient (clinic), inpatient (hospital) and nursing home settings. Together, working with you in partnerships, we will connect the dots needed to ensure the best possible care for you and your families.

Meet the doctor.
Umar I Chouhdry, MD, MBA
Dr. Chouhdry is a native New Yorker. After attending the prestigious Bronx High School of Science, Dr. Chouhdry attended Marymount Manhattan College before going on to receive his MD from St. George’s University School of Medicine. During the course of his residency training from Lutheran Medical Center in Brooklyn, NY, Dr. Chouhdry served as House Staff President, and won accolades for teaching excellence, patient care, and cultural competence initiatives.
Dr. Chouhdry is American Board of Internal Medicine certified and is also additionally trained as a Physician Executive, from the University of Tennessee's Physician Executive MBA program.
His main focus is on health care reform through the delivery of "Foundational Care" rather than "primary care," via a comprehensive approach which puts the patients first, and quality care delivery is enhanced through the use of technology.
As a New Vision doctor, Dr. Chouhdry is committed to becoming your "medical quarterback" helping guide you and your family through the complex playing field of modern healthcare.

Q: What if I don't have insurance?
A: At New Vision we believe in providing access to all patients independent of insurance status. We have reasonable payment options for those clients who do not have insurance. Contact our administrative assistant for details. We are happy to serve.
Q: What insurances will you be accepting?
A: We now accept Medicare, Empire Plan, Empire BCBS HMO, Cigna, Aetna, GHI HMO, GHI PPO, Blue Cross Blue Shield HMO/PPO/EPO, MVP, 1199, Wellcare, United ealthcare, and Hudson Health.
